INITIAL INTAKE FORM

Before your appointment

✓ Horse:

Please have your horse ready with a halter and lead in a quiet area. The horse should be dry and clean, including the feet. A flat, firm surface will be required for dynamic assessment. Do not feed your horse 1-2 hours before the visit.

Duration: ~1 hour and 15 minutes

✓ Dog:

Please have your dog ready with a collar and lead in a quiet area of your home away from other dogs and distractions. The dog should be dry, clean, and allowed to a toilet before the session. A flat, firm surface will be required for dynamic assessment. Do not feed your dog 1-2 hours before the visit.

Duration: ~1 hour

Please note, physiotherapy does not replace a veterinary diagnosis. In case of acute changes in condition, please, speak to your treating veterinarian to obtain a Veterinary Referral (2).

Veterinary Referral Form

For new patients, diagnosed conditions and acute cases. Although veterinary referral is not required by Luxembourgish legislation, it is highly recommended as it allows us to focus on the exact diagnosis and provide the best care and outcome possible.
